People with diabetes in Kent and Medway are now able to self-refer to an online educational tool tailored to support managing their condition.
The myDiabetes platform, provided by my mhealth, has around 1,000 people signed up to it in west Kent and Medway.
Now, anyone can use it without the need to be referred by a healthcare professional.
To self-refer, you will be asked to complete an application form, which asks name, date of birth, email, NHS number, contact number and which GP you are registered with. The my mhealth team will then be able to register you on the platform and you will receive an email with an activation link.
GP Clinical Lead for Diabetes in Kent and Medway Dr Rebecca Prince said: "The myDiabetes platform complements the care patients already receive from their designated healthcare professional.”

Global Transformation Lead for mymhealth Ian Thompson said: “We are proud to be part of the delivery for diabetes services across Kent and Medway.
“The self-referral pathway empowers people living with diabetes by giving them greater control on when they get access to what they need for their diabetes management.”
NHS Kent and Medway Clinical Commissioning Group (KMCCG) worked with the Paula Carr Diabetes Trust to introduce the platform to Kent and Medway in April 2021, and the people registered so far have watched 948 educational videos.

GPs and clinicians are still able to refer people with diabetes to the platform; it’s available online or through download on an iPhone or android.
